1. Volunteer Driver Authorization Forms are valid for one school year and must be completed annually.
2. Please be sure to fill out the form completely and sign and date in both required sections: Driver’s Signature and Registered Owner’s Signature.

Once you’ve filled it out, please give it to your child’s classroom teacher.
